I'm interested to find out what exactly is the issue for your problems.
The easiest way to restart the whole thing is....

mv /mnt/usb/install/auto_package_done  /mnt/usb/install/auto_package
mtd write -r /mnt/usb/openwrt-ar71xx-generic-tl-mr3040-v2-squashfs-factory.bin firmware

that will reflash the entire device and trigger the auto installation.

maybe something odd happend during the first flash of the device... this sometime occur :(
