yes, please start of the scratch with a fresh new flashed firmware.

as I recognized, you have it on the usb stick, so you can run

(piratebox)

cd /mnt/usb
ls openwrt*
mtd -r write <openwrt image file> firmware

(please replace <openwrt image file> with the name of the file)

and PLEASE PLEASE PLEASE enable the log-option in putty and capture the output during piratebox installation in a logfile. You find that under

  * Session
     +---> Logging

Enable "printable output"

And then please post the log file, each part of it before you overwrite or delete it. Without that I can't proove any issues
